Deposit Account Application
Thank you for choosing Putnam Bank for your financial needs. Here is how the account opening process works: First Step: Complete the Account Opening Application with all of the required information. Please note that you must be 18 years or older, must reside or work within the bank’s local community, must be a citizen of the United States or a Resident Alien and must provide a valid Social Security Number and Photo Identification information. Federal law requires all financial institutions to obtain, verify, and record identification information for each person who opens an account. For Non-Resident Alien individuals please visit one of our eight convenient branch locations. Second Step: Once you have submitted your deposit account application form we will begin to create a customer account for you. One of our Customer Service Representatives will contact you to set up a convenient time for your account opening appointment. Third Step: When you arrive at your appointment you will be provided with account disclosures and agreements and will sign bank documents. In order to add a joint owner or an additional signer to your account that individual will need to accompany you to your account opening appointment and please bring photo identification.
